    25羟维生素D3 [1,25-二羟维生素D3对大鼠肾间质纤维化的作用研究]

      [摘要] 目的:研究1,25-二羟维生素D3〔1,25-(OH)2D3〕对单侧输尿管梗阻(UUO)大鼠肾间质纤维化的影响。方法:45只大鼠随机分为假手术组(n=15)、UUO组(n=15)和1,25-(OH)2D3干预组(n=15),建立UUO大鼠肾间质纤维化模型,干预组于术后第1天起每天腹腔注射1,25-(OH)2D3,假手术组及UUO组腹腔注射等量生理盐水。分别于术后第3天、第7天和第14天处死大鼠,每次5只,取梗阻侧肾脏行HE、Masson染色及α-平滑肌肌动蛋白(α-SMA)免疫组化标记,观察肾损伤程度和肾间质纤维化程度;取血测定肌酐和尿素氮水平观察肾功能;处死前1天收集24 h尿液,检测尿中蛋白含量。结果:与假手术组相比,UUO组和干预组各时间点HE、Masson染色和α-SMA表达变化明显,呈逐渐加重趋势,差异有统计学意义(P<0.05);肌酐、尿素氮及24 h尿蛋白含量术后第3和第7天时表现为升高,术后第14天时出现下降,差异有统计学意义(P<0.05)。与UUO组相比,干预组各时间点各项指标变化明显改善,差异有统计学意义(P<0.05)。结论:1,25-(OH)2D3可抑制UUO大鼠肾间质纤维化进程,改善肾功能。

      The effects of 1, 25-dihydroxyvitamin D3 on interstitial fibrosis to kidney after unilateral ureteral obstruction in rats

      [Abstract] Objective: To investigate the effects of 1, 25-dihydroxyvitamin D3 [1,25-(OH)2D3] on interstitial fibrosis to kidney after unilateral ureteral obstruction (UUO) in rats. Methods: 45 rats were randomly divided in to 3 groups: sham operated group (n=15) which was given 1, 25-(OH)2D3 by intraperitoneal injection one day on post operative every day, UUO group (n=15) and 1, 25-(OH)2D3 treatment group (n=15) which were all given normal saline by intraperitioneal injection equivalently. They were sacrificed at the 3rd, 7th and 14th days respectively after UUO, every time for only 5. Interstitial fibrosis of obstructed kidney was graded according to HE and Masson staining. The expression of α-SMA was examined by immunohistochemistry. The samples blood were collected to detect the concentration blood serum creatinine and blood urea nitrogen. Daily urinary protein were determined in these rats one day before sacrificed. Results: The degree of interstitial impairment and fibrosis of obstructed kidney and the expression of α-SMA in the UUO group and treatment group were significantly higher than those in the sham operated group (all P<0.05). The blood serum creatinine, blood urea nitrogen and daily urinary protein in the UUO group and treatment group were increased at the 3rd and 7th days after UUO, but they were decreased at the 14th day (all P<0.05). Compared with UUO group, all which in treatment group were significantly decreased (all P<0.05). Conclusion: 1, 25-(OH)2D3 can alleviate interstitial fibrosis and renal function in rats after UUO.

      肾间质纤维化(renal interstitial fibrosis,RIF)是一个多步骤、连续的动态过程,是各种慢性肾脏疾病发展至终末期的共同通路和主要病理改变[1],其变化的程度和范围决定了肾功能的恶化程度[2-4]。单侧输尿管梗阻(unilateral ureteral obstruction,UUO)大鼠是一种经典的RIF动物模型,既可短期内致使肾间质纤维化,模拟慢性肾病时肾间质的病理表现,又可使病变局限在肾小管和肾间质,不引起肾小球及其他组织的损伤[5]。近期研究显示,1,25-二羟维生素D3〔1,25-dihydroxyvitamin D3,1,25-(OH)2D3〕主要通过与其特异性受体结合发挥经典的骨盐调节作用,并参与免疫调节、调控激素分泌和细胞有丝分裂、抑制细胞增殖、分化及凋亡等过程[6]。1,25-(OH)2D3受体可选择性表达于肾脏的近曲小管、远曲小管、集合管和髓袢升支[7],但其是否在RIF形成过程中发挥作用不详。本研究利用1,25-(OH)2D3干预UUO大鼠,观察其对RIF形成有无影响,以探讨其在RIF过程中的作用。
      1 仪器与试药
      1.1 实验动物
      雄性SD大鼠,45只,8周龄,体重(200±20) g,购自泸州医学院医学实验动物中心。
      1.2 药物和主要试剂
      1,25-(OH)2D3(Roche公司,美国),兔抗大鼠α-SMA多克隆抗体(Santa Cruz公司,美国),尿蛋白双缩脲试剂盒(Amresco公司,美国)。
      2 方法与结果
      2.1 实验方法
      2.1.1 实验动物分组及处置 45只SD大鼠随机均分为假手术组、UUO组和1,25-(OH)2D3干预组,每组各15只。采用单侧输尿管结扎建立UUO大鼠模型[8]。1%戊巴比妥钠腹腔注射麻醉后,局部剃毛和常规消毒铺巾,选腹正中切口依次切开皮肤至腹腔,游离肾脏及输尿管,分别在左侧输尿管近肾盂处和输尿管上1/3处用1-0丝线两次结扎,剪断输尿管,然后逐层连续缝合皮肤。假手术组进入腹腔后仅游离肾脏和输尿管而不结扎离断,余手术方式与UUO组相同。干预组于术后第1天起每天腹腔注射30 ng/(kg·d)的1,25-(OH)2D3[9],假手术组及UUO组每天腹腔注射等量生理盐水。分别于术后第3、7和14天处死大鼠,每次5只,取梗阻侧肾脏测量其大小及形态,固定包埋后,制片行HE、Masson和α-SMA染色;开胸经心脏取血检测肌酐和尿素氮水平;各组大鼠于处死前1天收集24 h尿液,检测尿蛋白含量。
      2.1.2 肾脏病理检查 每张HE、Masson和α-SMA切片随机选取10个不重复视野(×200,HPF),组内求和取其平均值作为每组最终数据。①HE染色判定肾间质损伤及纤维化程度:HE切片依据Radford[10]标准设定8个参数指标(肾小管扩张、小管萎缩、小管上皮细胞空泡变性、间质水肿、间质炎细胞浸润、间质纤维化程度、红细胞管型、蛋白管型),每个参数按0~3分评定(0分,正常;1分,轻度受损,病变范围<15%;2分,中度受损,病变范围15%~50%;3分,重度受损,病变范围>50%),由两个观察者盲法评分取其平均值,每个样本的评分为0~24分。②Masson染色判定纤维化程度:Masson染色是用于显示组织中纤维的一种染色方法,染色切片中胶原纤维呈绿色,肌肉、纤维素和红细胞均呈红色,细胞核呈蓝色,正常肾组织中仅肾小管基底膜染成绿色。RIF进程中胶原纤维逐步取代正常肾单位,肾实质萎缩减少。本实验通过对胶原纤维染色,运用Image Pro Plus 6.0图像分析软件计算镜下绿染面积占整个视野的百分比,以了解受损肾脏的纤维化程度。③α-SMA免疫组化染色:在本实验中α-SMA用于标记肌成纤维细胞(myofibroblast, MFB),其阳性信号为细胞胞浆呈现棕黄色颗粒。运用Image Pro Plus 6.0图像分析软件计算染色阳性细胞的积分光密度值(指将阳性细胞内染色量化为综合面积和灰度值所得到的结果,其值越大,提示抗原表达水平越高,反之亦然)。
      2.1.3 肾功能检测 采血后3 000 r/min离心分离血清,于日本7180型全自动生化分析仪上检测血肌酐、尿素氮水平(此血生化检查由泸州医学院附属医院检验科协助完成);根据尿蛋白双缩脲试剂盒的操作说明,检测24 h尿蛋白含量。
      2.1.4 统计学方法
      用SPSS 17.0统计软件进行统计学处理,所有数据采用均数±标准差(x±s)表示,多组间计量资料比较用单因素方差分析(One-way ANOVA test),两两比较采用Dunnett-t检验。以P   本实验结果显示,随着时间延长,UUO组肾间质损伤和纤维化程度逐渐加重,α-SMA标记阳性的MFB逐渐增多;至术后第14天,肾间质纤维化明显,α-SMA表达达到高峰。而肌酐、尿素氮和24 h尿蛋白水平于术后第3、7天与梗阻侧肾脏损伤相平行,各项指标明显升高;至第14天,各项指标明显改善,这可能与实验所取血液为全身血液,而此时健侧肾脏发挥了全面代偿作用有关。干预组与UUO组各时间点相比,干预组各项指标改善明显。另有体外实验证明,1,25-(OH)2D3能抑制转化生长因子-β1(tranforming growth factor-β1,TGF-β1)诱发的α-SMA表达,并呈剂量依从关系[15]。TGF-β1是目前已知最强烈的促纤维化因子,可诱导EMT和成纤维细胞活化;而1,25-(OH)2D3 的体内信号传导途径与TGF-β1存在有广泛交互的作用[16]。1,25-(OH)2D3在RIF中发挥保护作用的可能机制是通过拮抗TGF-β1诱导的EMT和减少蛋白尿而实现的。
      RIF是一个多因子参与、多路径调控的复杂病理过程,其机制还有待进一步的研究。本研究对1,25-(OH)2D3在RIF中所起的干预作用作了初步探讨,为进一步研究RIF发病机制及寻找治疗方法提供了一定证据和思路。
